POLICY: Assessment and service planning is an ongoing, collaborative, and continuous process at the Company. This process is a mutual exploration of the client's issues and strengths, complemented by the staff's professional perspective and recommendations resulting in a jointly created plan with mutually agreed-upon goals.

SCOPE: This policy applies to all Company employees involved in direct service.

Procedures

  • Assessment begins in the initial interview (The client intake and assessment process is further outlined in Appendix 3 - Client Intake Questionnaire.) and builds on the information and presenting issues gathered during intake.
  • Include the template of the behavior plan used at the Company (Note: this can include only the tabs if preferred.)

Service planning includes ongoing review of progress and consideration of discharge readiness when clinically appropriate. Discharge criteria, transition planning, coordination of care, and documentation requirements are detailed in Appendix 24 - Discharge and Transition of Services Policy.
