Client Bill of Rights

All individuals receiving services from the [Company] and their families are entitled to the following rights. The [Company] is committed to providing services in a respectful, safe, and ethical manner while protecting the dignity and well-being of every individual.

Rights of Every Client

Each client has the right to be treated with dignity, respect, and courtesy at all times, without discrimination based on race, color, ethnicity, religion, gender, gender identity, sexual orientation, national origin, disability, age, or socioeconomic status.

Each client has the right to receive services in a safe, clean, and supportive environment that promotes well-being and personal growth.

Each client has the right to be fully informed about the services provided, including the purpose, procedures, potential benefits, and possible risks.

Each client has the right to participate in the development and review of the service or treatment plan, including the right to ask questions and express preferences regarding services.

Each client has the right to refuse or discontinue services, to the extent permitted by law and program requirements, and to be informed of the potential consequences of such decisions.

Each client has the right to receive services from qualified, trained, and supervised professionals who follow ethical and professional standards.

Each client has the right to privacy and confidentiality of personal information in accordance with applicable federal and Florida laws, including the Health Insurance Portability and Accountability Act, except where disclosure is required or permitted by law.

Each client has the right to review, request amendments to, and obtain copies of records in accordance with organizational policy and applicable laws.

Each client has the right to be free from abuse, neglect, exploitation, coercion, retaliation, humiliation, or harassment of any kind.

Each client has the right to receive services that are culturally responsive and respectful of individual values, beliefs, and family traditions.

Each client has the right to be informed of organizational policies, rules, and expectations related to receiving services.

Each client has the right to voice concerns, complaints, or grievances regarding services, staff conduct, or organizational practices without fear of retaliation.

Each client has the right to have complaints or grievances reviewed promptly and fairly in accordance with the organization's grievance procedures.

Each client has the right to receive coordination of care with other professionals or agencies when appropriate and authorized.

Each client has the right to be informed of any fees, billing practices, or financial responsibilities associated with services.

If a client or parent or guardian believes that any of these rights have been violated, they may file a complaint through the organization's grievance procedure.

I acknowledge that I have received and reviewed the Client Bill of Rights for services provided by the [Company].
