Clinical Progress Discharge Summary

This form documents the client's clinical status and progress at the conclusion of ABA services. It records the areas of progress, the status of targeted maladaptive behaviors from intake to discharge, skill acquisition at discharge, caregiver participation, recommendations, and the overall clinical impression. It is completed by the supervising behavior analyst and may be used together with, or in place of, the Discharge Notification Form, depending on the documentation a clinic already maintains.
