Receipt of Employee Handbook and Employment-At-Will Statement

I acknowledge that I have received and read a copy of the Company's Employee Handbook. I understand that the Employee Handbook sets forth the terms and conditions of my employment with the Company, as well as the duties, responsibilities, and obligations of that employment. I understand that the Company has provided me with various channels, including confidential channels, to raise concerns about violations of this Handbook or Company policies, and encourages me to do so promptly so that the Company can effectively address such situations. I further understand that nothing in this Handbook interferes with my right to report concerns, make lawful disclosures, or communicate with any governmental authority regarding potential violations of laws or regulations.

I agree to abide by and be bound by the rules, policies, and standards set forth in the Employee Handbook.

I acknowledge that my employment with the Company is at-will. Florida is an at-will employment state, which means that my employment is not for any specified period of time and that the employment relationship may be terminated at any time, for any reason, with or without cause and with or without notice, by either me or the Company. I further acknowledge that only the President/Owner or his or her authorized representative has the authority to enter into any agreement that alters my at-will employment status, and that any such agreement must be in writing and signed by the President/Owner or that authorized representative.

I understand that the Company reserves the right to revise, delete, or add to the provisions of the Employee Handbook, and that any such changes must be in writing. No oral statements or representations can change the provisions of the Employee Handbook. The Company's policy of at-will employment may only be changed in the manner described in the prior paragraph.

I understand and acknowledge that nothing in this Employee Handbook or in any other Company document or policy is intended to prohibit me from reporting concerns to, filing a charge or complaint with, making lawful disclosures to, providing documents or information to, or participating in an investigation or hearing conducted by the Equal Employment Opportunity Commission (EEOC), the National Labor Relations Board (NLRB), the Securities and Exchange Commission (SEC), the Florida Commission on Human Relations, or any other federal, state, or local agency charged with the enforcement of any laws.

I also understand and acknowledge that nothing about the policies and procedures set forth in this Employee Handbook should be construed to interfere with any employee rights provided under state or federal law, including Section 7 of the National Labor Relations Act.

I further certify that I have submitted all onboarding and compliance documents requested by the Company, including but not limited to Form I-9, Form W-4, the Confidentiality and HIPAA agreements, background check authorization, proof of credentials, and any required acknowledgments contained in this Handbook.
