Initial Job Application
Pre-employment application form for staff and contractors.
This is the pre-employment application every applicant completes before [Company] will consider them for a position. It captures the applicant’s identifying information, employment history, work authorization, emergency contacts, and the authorizations [Company] needs to verify references and background.
It is our policy to comply with all applicable state and federal laws prohibiting discrimination in employment based on race, age, color, sex, religion, national origin, disability, or other protected classifications. Every question on the application must be answered completely. A résumé may be attached, but does not replace the answers on the form.
The Initial Job Application form on the right captures every required field, including the at-will employment acknowledgement, the applicant’s signed certification, and the authorization initials that allow [Company] to investigate references and prior employers. Fill it in the browser during intake, or download a blank copy for the applicant to complete in person. The signed application is filed in the applicant’s pre-hire packet and, if hired, moves into the employee’s HR file.
