Required Documents Checklist
Required employment documentation and verification procedures.
HR runs this checklist for every new provider during onboarding. It tracks every document, background verification, credential, and training that must be on file before the provider’s first day.
The Required Documents Checklist form on the right captures each item in one place: applicant details, HR verifications, NPI and insurance participation status, the documents collected, and the trainings completed. Fill it directly in the browser as items come in, or download a blank copy to walk through during an in-person review. The completed form is stored in the applicant’s HR file.
Several of the required trainings are completed online through Florida’s free training portal at www.fl.train.org. These include HIV/AIDS CEU, ZERO Tolerance, HIPAA Training, OSHA Bloodborne Pathogens, and Security Awareness Best Practice. CPR / AED / First Aid and Direct Care Core Competency are completed through approved outside providers. Save each completion certificate in the applicant’s HR file alongside the checklist.
