Under the Company's current plan, full time employees may enroll in a single, single plus one dependent, or family contract for medical and dental insurance on the first day of the month following four consecutive payroll periods of working 60 or more hours (at least 30 hours per week). The Company currently contributes 75% of the employee's medical premium and 15% of dependent and family medical premiums, with the employee responsible for the remainder. The Company currently contributes 75% of the employee only dental premium, with the employee responsible for the remaining 25% as well as 100% of any premium for family dental coverage. Premiums that are the employee's responsibility are paid through automatic payroll deduction.

Information and enrollment forms are available from Human Resources. Participating employees are also covered under the prescription drug program included with the medical plan.
