The Company maintains all original patient records at its headquarters office, with electronic copies stored on Company servers. Records are retained for at least six years in accordance with the HIPAA Privacy and Security Rules. Patient records are maintained exclusively in electronic format consistent with HIPAA requirements.

Every patient record is confidential. Records may not be released to any outside agency without a signed authorization from the minor patient's parent or legal guardian.

Patient records are audited at least twice per year to confirm that all required documentation is present and current.

Privacy Statement

The Company treats every piece of information obtained from or about patients in the course of providing services as confidential. The Company does not share or distribute that information to third parties except as authorized and necessary to deliver services and assist the patient. Employees may not provide such information to any third party without first receiving authorization from their supervisor, after a written request from the third party and a signed, notarized release of information from the patient (or the patient's parent or legal guardian) whose information is being requested.
